There is no way to write a synopsis of this issue that wouldn’t be longer than the comic itself.  It’s chock full of characters I swear I’ve never seen before, all of them out to get Frank Castle.  Why?  Uhhhh.  Plus, all his formerly dead friends are in danger of being killed.  Again.  It’s like Frank Castle’s Blackest Night if it were written by a cracked out college freshman majoring in fanfic.

By the end of the issue it was no longer a case of not knowing what was going on, it was more a problem of not caring.
